Brief Paperwork
Upon entering the office, we have some brief paperwork for you to fill out. The paperwork provides us with general information about yourself and your condition. You may save time by dowloading the following
forms, filling them out and bringing them with you for your first
appointment.

Directions
Click the above link(s) and save the files to your desktop. These are PDF files, created for viewing in Adobe Reader. You may click on them at your desktop to open them up in Adobe Reader (see below if you don't have the software).

To fill out the form(s) by hand...
Print the form(s) immediately to your printer using non-glossy paper.

To fill out the form(s) on your computer...
Select the "Highlight Fields" checkbox in the upper-right corner of the open document window, inside the Adobe Reader program. Doing this will show you the editable areas of the forms. Fill in as many areas as you can (use the TAB key to move from text field to text field, or SHIFT-TAB to move backward). Save the document periodically. That way, if you need to come back and finish the form later, it will preserve all of the information you typed. When you are finished, simply print out the form on your printer.

Consultation
Once the paperwork has been completed, you will have a personal consultation with Dr. Deb to discuss your health problems, issues, concerns, and treatment options. Just like any other medical office, we are bound to confidentiality and privacy.

History and Examination
In order to determine what your health problems are, and where they might come from, Dr. Deb will ask you various questions in order to determine the best path(s) for treatment. Next, a number of simple tests will be performed to determine the extent of your condition(s) and to get other related health information.

During your first appointment, you'll also be introduced to the office, its layouts, its philosophy, different modes of treatment, and testimonials from both current and former patients. Once finished with the examination, patients will be asked to schedule a time for their first treatment. It's important that the doctor carefully review your medical history, current findings and test results. (Treatment immediately after the initial evaluation is not conducted in our office.)

Then you'll arrange a convenient time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time. Payment arrangements are often discussed during the follow-up visit as well.

X-rays
Many health conditions are critical enough to warrant taking x-rays to either rule out more serious conditions, or assist us in developing the most effective treatment plan for you.

Home Instructions
Prior to leaving from your first visit, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.
